Bloodshed is a rune effect in Battle Rage, a Fury-spending Barbarian skill.
This rune effect is yet another in this skill that provides a special bonus for landing critical hits. This one adds a powerful AoE effect, causing an explosion of blood with every critical hit, a splash of gore that deals 20% of the damage done to all other nearby enemies.
A Barbarian of at least level 54 will tend to deal thousands of points of damage with every hit, so this is a considerable amount of bonus damage. This also encourages the Barbarian to fight mobs in tight groups, so that as many enemies as possible are in the unspecified "close" range.
