Focused Mind is a rune effect in Preparation, a Discipline-spending Demon Hunter skill.
This rune effect grants a steady increased regeneration of the Discipline resource, filling up 3 points per second over 15 seconds, for a total or 45 points.
A Demon Hunter will seldom have more than 30 Discipline, unless the total is boosted considerably by equipment, so this effect will usually grant more than a total refill. Hence some Discipline should be expended in beneficial fashion during the 15 second refilling, lest it go to waste.
The same 45 second cooldown remains between uses.
