Shoulder armor is a new type of armor added in Diablo III that was not present in Diablo 2. Shoulders are worn between the arms and the neck, partially overlapping the body armor.
Blizzard's artists are notorious (largely from World of Warcraft) for including ridiculously-oversized shoulder pads on their characters. Diablo III isn't quite following that path, but some of the early Barbarian and Monk gear isn't exactly subtle in the shoulder ornamentation.
Known Shoulder Types
The only kind of shoulder armor yet named in Diablo III are Pauldrons.
