Special Modifiers were a group of item modifiers in Diablo II that were not prefixes or suffixes. Special Modifiers only occurred on Crafted items, Set items, and Uniques; never on magical or rare items. These type of modifers grant unusual bonuses; properties very different from the vast majority of prefixes and suffixes.
It's not known if Diablo III will have affixes that are classified as special modifiers, since the developers have revealed very little about the itemization process.
