The Berserker was initially thought to be a larger, mutated human. A closer look has shown them to be outright demonic beasts, with huge fangs and monstrous faces. They serve Cultists as enslaved enforces, bound to the dark will of their human masters.

Beserkers are pure melee brawlers, armed with huge maces and a ferocious blood lust. Their trademark attack is a devastating two-handed smash with their club.

Berserkers are the muscle of the Cultists' evil, demon-worshipping cult. They are massive, powerful men who wear hoods and loincloths and carry huge mauls, with which they are happy to bash your brains in.

Berserkers have a special overhead smashing attack, which they telegraph by powering up their mauls with a reddish glow. Their overhead attack is highly damaging, but if you dodge it the Berserker will hit the floor so hard their maul breaks through the stone and gets stuck there. While the Berserker struggles to pull his weapon free, he is defenseless and can be killed swiftly.

It's not yet known if Dark Cultists or some other caster-type monster can cast any buffs on the Berserkers.

Background

It's likely that Berserkers are part of the Cultists cult, and either are just extraordinarily strong, or turned into strong, mindless slaves by demonic corruption.

Development

Triune Leader Blood Guard concept.

The Berserker was first shown at WWI 2008 when Diablo III was first announced. It has not changed much since then, but it seems there either are variables to it, or a boss type monster associated with this monster: The Triune Leader Blood Guard.

BlizzCon 2010 Upgrades

Berserker Champions.

There were Berserkers seen in the Blizzcon 2010 PvM demo, and they were huge. Only three of them were present, but they were always found in the same place, near the end of the Highlands Passage, where they defended a quest boss Cultist from the Mad Weaponmaker.

These three Cultists were huge; at least double their usual human size, but it's not clear if this is was a change in the monster type, making them larger and more impressive, or if it was purely due to those three being Champions, and thus larger than normal.
