Report from Wortham
The Report from Wortham can be found on the corpse of a Wortham villager at the very north of the Northern Highlands in the courtyard just before entering Leoric's Hunting Grounds. You are not required to be on any specific quest for his corpse to appear.
Only one of your characters needs to find the journal for you to be credited with it.
To find out which journals you have found so far go to your Achievements list (press Y) and check:
