Despina's Journal is found inside Old Keepsake Boxes which can be found on the ground along the Old Tristram Road or within The Old Ruins. This map shows you where the boxes can spawn.
Only one of your characters needs to find the journal for you to be credited with it.
To find out which journals you have found so far go to your Achievements list (press Y) and check:
