It can be found if you loot the body of a fallen adventurer on the ninth level, the Caves. As it can't be read, just picking it up will initiate the [[{{iw|Black_Mushroom Black Mushroom]] }} quest. Next the players need to speak with [[Adria]].
Author of the book is unknown, but seeing as it's located on ancient [[Horadrim|Horadric]] caves, it's likely one of the ancient mage order.
