It can be found if you loot the body of a fallen adventurer on the ninth level, the Caves. As it can't be read, just picking it up will initiate the [[Black Mushroom]] quest. Next the players need to speak with [[Adria]].
Adria says it's filled with all kinds of reagents used in potions and elixirs, most notably the Black Mushroom.
Author of the book is unknown, but seeing as it's located on ancient [[Horadrim|Horadric]] caves, it's likely one of the ancient mage order.
