Tunnels of the Rockworm can sometimes be found in the Deserted Cellar in the Black Canyon Mines of Act II. The entrance to the tunnels does not always appear in the cellar, sometimes a frightened girl, Larra does and other times it can be a ritual that ends in a battle with a superunique called Graveljaw the Devourer.
The tunnel entrance will appear in the Deserted Cellar if Graveljaw the Devourer surges out of the ground. Once he's dead you can descend down the tunnel he formed.
