There is more than one meaning to Hell in the Diablo IIIgame s. If you were linked here by an article pointing to one of these meanings, you should edit it to point directly to the intended meaning. Hell in Diablo games can mean one of the following:
Location: Hell
The Burning Hells is the birthplace of demons like three Prime Evils of Diablo, Mephisto or Baal. They are in conflict with the High Heavens, where Angels like Tyrael rule.
Game Difficulty: Hell
Difficulty settings in Diablo III are planned to work very similar to Diablo II. When starting your first character in Diablo II, you will automatically have a character starting on The 'Normal' difficulty setting. This difficulty setting dictates monster hit points, damage, immunities etc. It's mathematically impossible for a starting character to survive in any other difficulty setting than Normal (if it was possible to do that). When a character kills Diablo, he can then continue to the difficulty difficulty, playing the game again. When Diablo is vanquished on Nightmare setting, a last testament on Hell difficulty can start. "End-game" things like is the hardest Pandemonium Eventdifficulty or killing Uber Diablo can only be done setting in Hell difficulty in the Diablo IIgames.
Diablo III Difficulty settings will probably be quite similar to Diablo II.
