Some legendary items have another primary affix, breaking the usual 4/2 item form with an extra affix, or some unusual special property (often +Elemental Damage) as a primary attribute. These are often amongst the most powerful items available. Some were in place with five affixes from back in the D3v days, others were upgraded to five primary for the launch of RoS, and other items were granted a fifth primary affix in post-release patches. (Patch 2.1 added half a dozen to the list.)

Note that this does not refer to Legendary Affixes, which are indicated in orange text and which the game always classifies as secondary affixes.
